If I remember correctly, Scott Monty was the first to use the term tweet-up, circa May 2007. (He has his original usage bookmarked.)

While most tweet-ups are social occasions, a new trend is to hold tweet-ups in conjunction with a social cause, i.e. the Austin Blood Drive Tweet-up, which produced a record turnout and the highest number of first-time blood donors for the Central Texas Blood and Tissue Center, and the recent Vancouver Tweet-up Heat-up, an impromptu gathering of Twitter friends to bring coats and warm clothing to the homeless.</description>
